Hospice Medical Director
The hospice physician/medical director leads the Hospice Care team in developing a plan of care to meet each patient’s specific needs. They consult with physicians about hospice care and certify terminal prognosis as part of the hospice eligibility process.

Registered Nurse
When a patient has been diagnosed with a life-limiting illness, our compassionate team of hospice nurses are there to meet their needs. Hospice nurses assess and manage a patient’s pain and symptoms. They provide comfort to the family and teach them how to care for their loved one.

Wound Care Nurse
These nurses can provide special care for wounds and ostomies. They will recommend supplies to help manage patient’s wounds.

Behavioral Health Nurse
They will identify safety risks, recommend medications and teach coping skills. These nurses work with patients to sort out the feelings that affect them and their families facing terminal illness and death.

Certified Nurse Aide
Hospice aides are certified nursing assistants. They help patients with personal care and everyday activities. They may help patients bathe, get dressed and prepare meals.

Physical therapist
A physical therapist can work with a person in hospice care to:

  • Determine a patient’s ability to move around safely.
  • Improve a patient’s ability to get in and out of bed or chair.
  • Help patients be more independent.
  • Adjust the home environment to better fit a patient’s needs.
  • Reduce the burden on caregivers and care team members.
  • Help patients safely use the bathroom and bathe.

Speech-language therapist
These therapists aim to help you communicate, evaluate your swallowing ability and improve your understanding. This will help you feel more comfortable and satisfied with your eating.

Occupational therapist Occupational therapists evaluate your needs. They identify barriers that keep you from engaging in activities such as:

  • Everyday activities
  • Rest and sleep
  • Social activities
  • Leisure activities
  • They also determine your need for special equipment.

Dietitian
A registered dietitian is available for counseling, teaching and evaluation. They also make home visits or phone consultations. They can help your loved ones honor your food or beverage preferences. In addition, they can help you make changes to ensure you are safely consuming food or fluids.

Medical Social Worker
Social workers give information and resources to help you stay safe and cared for where you live. The social worker helps set up community resources to help with any medical, psychological, financial or social assistance that you need.

Hospice Chaplain
Our spiritual professionals can support patients and loved ones. They support your beliefs, regardless of culture or religion.

Massage Therapist
Massage therapists can bring comfort and relief to those with life-limiting illnesses.

Volunteers
The end-of-life journey can be hard for loved ones. Volunteers are available to sit with patients, read to them, or spend time with them. Our volunteer coordinators match the volunteers with patients to meet that patient’s needs.
