Making an Appointment with a Chordoma Specialist

To make an appointment at the Lou and Jean Malnati Brain Tumor Institute at Northwestern Memorial Hospital, call us 24/7 at 312.695.4360 or fill out a form.

You may be asked to bring the following items with you on the day of your first appointment, or to send them to our office before your appointment:

• Copies of your written medical records so that our physicians have a complete view of your medical history
• Pathology slides and reports
• Copies of any MRIs or CT scans (these can be obtained from the imaging department where they were taken)
• List of current medications

During your appointment, your physician will review these materials with you and offer recommendations for treatment. Your physician will work with you to make sure your diagnosis is understood and all of your questions are answered.
