Home health care aims to treat and educate patients and loved ones on an illness, injury or skilled service. It can improve your health, promote independence, identify safety needs and concerns, and help prevent readmissions.
Adults 18 and older who:
- Have a clinician’s order
- Are considered homebound
- Are needing intermittent skilled services
To be considered homebound, the following must be true:
- You need help from a person or medical equipment (such as crutches, a walker or a wheelchair) to leave your home. Or, your physician believes that your health or illness could get worse if you leave your home.
- It is hard for you to leave your home, and you typically cannot do so.
If you going home from a hospital or a rehabilitation facility, the discharge planner, social worker or case manager can help you arrange home health care.

Medicare Part A covers home health services when you meet the criteria. Most health insurance plans cover some of the cost. Check your health plan to learn about your coverage.
