Application Requirements

  • Resume
  • Written personal statement
      • In 500 words or less, describe why you are interested in applying for the OT Acute and Critical Care Fellowship at Northwestern Memorial Hospital and how it will impact your long-term professional goals
    • Two letters of recommendation
      • One from a clinical instructor from an acute care rotation or supervisor from previous acute care employment
      • One letter from a professor or from an occupational therapy employer if you have been out of school for more than two years
      • Note: Include both letters of recommendation in your application. If the individuals writing your letters prefer to submit them directly, have them email the letters to nmotfellowship@nm.org.

    Fellowship Program Requirements

    You must have completed a student rotation in acute care or have acute care experience.

    If you are selected for the fellowship, you must submit all of the following:

    • AOTA membership (must have within 90 days of starting the fellowship)
    • NBCOT-certification or proof that you are eligible to take the NBCOT exam by June 15
    • Active Illinois occupational therapy license or proof that you will apply for an Illinois OT license by July 1
    • CPR/BLS certification

    Interview Process

    After we review your application, we will select potential fellows for an in-person or virtual interview. The interview will include:
    • A case study review
    • Interviews with these team members:
      • OT manager
      • OT coordinators
      • Mentor/staff panel
