Our Services

We serve adults 65 and older. Our team can:

  • Diagnose and treat mild cognitive impairment and dementia, including:
    • Alzheimer’s disease
    • Vascular dementia
    • Lewy body dementia
    • Parkinson’s disease dementia
    • Frontotemporal dementia
  • Offer resources and support.
  • Help with advance care planning.
  • Manage and treat behavioral disturbances in dementia.

How Does the Virtual Visit Work?

Appointments will be a video call on your tablet, smartphone or computer. You and a caregiver each meet with the team member.

During the call, we will:

  • Review your medications, focusing on ones that are high-risk for older adults.
  • Assess your fall risk.
  • Provide cognitive testing.
  • Review and order diagnostic imaging if needed.
  • Evaluate you for any other health issues common in older adults.
