Mental Health Case Management with Ben Gordon Center
Mental health case management services include assessment, planning, coordination, support, and advocacy services for clients who need assistance with:
- Gaining access to mental health, social, vocational, and educational services
- Housing
- Public income entitlements
- Identifying and investigating available resources
- Exploring options
- Linkage with or referral to necessary resources
- Other community services
Case management activities may include:
- Identifying and investigating available resources
- Building skills for daily living
- Explaining options to the client
- Client-centered consultation between the Providing staff member and other professional staff to improve client’s progress/outcome
- Linking clients with necessary resources
Our services are directed by a multidisciplinary team including:
- Board-certified psychiatrists
- Advanced practice nurses
- Registered nurses
- Licensed clinical psychologist
- Licensed clinical social workers
- Licensed clinical professional counselors
- Marriage and family therapists
- Addictions counselors
- Certified recovery support specialists
- Family partnership professionals

- Case managers
- Wellness Recovery Action Plan (WRAP) facilitator
