The Northwestern Medicine Lung Transplant Program is committed to providing comprehensive, compassionate care. When you refer a patient to our program, you become a part of the care team, along with our experts in the fields of advanced lung disease.

Together, we will develop a care plan that best meets your patient’s needs. We will maintain open communication throughout the patient’s care journey.

• Demographic sheet with patient’s insurance information
• Two to three days of recent progress notes
• The referring physician’s name and phone number to facilitate the physician-to-physician call
