Inpatient Admissions

For inpatient admissions, please call 630.909.8920 or the nurse liaison at 630.909.8882. The information listed below may be faxed to the Admissions Department at 630.909.8571.

  1. Face sheet
  2. History and physical, as well as a rehabilitation (PMR) consult, if available
  3. Progress notes and other consults as appropriate
  4. Therapy evaluation and notes
  5. Labs and vitals from the last 72 hours
  6. Medication list 

Outpatient Therapy and Day Rehabilitation

To refer a patient for outpatient therapy and day rehabilitation, please call 630.909.7150.

Physician Evaluation and Assessment Clinic
