The Boys & Girls Clubs of Dundee Township offer youth programming in education and career development, character and leadership, health and life skills, sports, fitness and recreation, and the arts.

With support from Northwestern Medicine, the organization has been able to run summer programming, including engaging physical activities, outdoor education and academic support. The program provides healthy meals and snacks every day, and offers a secure environment that prioritizes social and emotional learning and trauma-informed care as essential elements in all programming and staff training.

DeKalb County Community Foundation supports organizations and communities throughout DeKalb County by proactively addressing their needs and expanding, managing and distributing philanthropic resources.

The organization pooled financial assistance from Northwestern Medicine and other donors to purchase a Mobile Health Unit for the DeKalb County Health Department, which will use it to deliver critical health services to DeKalb County communities where many residents have barriers to care.

Elderwerks helps find assisted living or any type of senior housing or elder care solutions. They support the transition from one home to another, or help find the services one might need to remain in their home.

With financial assistance from Northwestern Medicine, Elderwerks offered the Elderwerks Aging Better Senior Expo, a virtual event offered at no cost for seniors and their families to learn how to plan for their futures regarding health, finance, care, benefits and living.

Erie Family Health Center (Erie) was founded in 1957 as a project of volunteer physicians from Northwestern Memorial and Erie Neighborhood House. Erie provides a variety of primary care, case management and dental services to over 37,500 low-income, underinsured and uninsured patients per year.

Erie’s eleven community-based health centers are portals to culturally sensitive, high quality healthcare—regardless of a patient’s ability to pay.

Based in Crystal Lake, Illinois, The Family Health Partnership Clinic works to better the health of McHenry County communities. With compassion and respect, the organization improves access to care by providing a broad spectrum of services to residents who experience barriers to health and wellness.

Support from Northwestern Medicine has helped cover general operating expenses for the clinic.

In partnership with McHenry County College, the Friends of McHenry County College Foundation raises funds on behalf of the college to benefit both students and programs.

With support from Northwestern Medicine, the foundation launched Education to Empowerment (E2E), an exciting women's scholarship program that gives financial support and ongoing mentorship for female students completing their final year at McHenry County College and continuing onto a four-year college or university. Funds from Northwestern Medicine and others will enable the foundation to continue to operate the program with promotion, scholarship events, mentoring events and member recruitment.

Hope Haven of DeKalb County provides programs and services to those experiencing homelessness to help them increase their wages, move to affordable housing, reduce debt and more. In the context of the COVID-19 pandemic, services like this have never been more critical.

Support from Northwestern Medicine has helped Hope Haven provide emergency shelter, food and supportive services, including case management, therapy, psychiatry, nursing care, life skills training, and employment and housing assistance.

Kelly Hall YMCA offer trusted health resources and programs that will help community members take action. These programs provide the tools and support needed to eat better, move more, and live healthier, including:

• Free fitness classes
• Nutrition and cooking classes
• Health education events
• Health screenings (including blood pressure, diabetes, and weight assessments)

With support from Northwestern Medicine and others, the McHenry County Mental Health Board provides leadership to ensure the prevention and treatment of mental illness, developmental disabilities and substance abuse by identifying, planning, coordinating, fostering development, and contracting for quality services for all citizens of McHenry County.

Financial support from Northwestern Medicine has been used to offer a full-day instructional conference at no cost for professionals and the community to discuss concerns, successes and challenges, and share tools and best practices with all who attend.

Near North Health Service Corporation (Near North), our community health partner for more than 40 years, provides comprehensive primary care, social services and dental services to primarily low-income uninsured or underinsured residents who live in some of the city’s most medically underserved communities.

To address increasing community needs, Near North expanded to provide clinical and social services through eight health centers, making it one of the largest providers of community-based primary care in Chicago. In fiscal year 2013, Near North served 46,130 patients through more than 102,018 visits.

Based in Crystal Lake, Illinois, Veterans Path to Hope works to uncover and provide the physical, emotional and other life needs that veterans face. From counseling to employment assistance to transitional living, the organization offers programs and services to help empower veterans.

Support from Northwestern Medicine has been dedicated to the on-site food pantry, which helps provide food to hundreds of individuals from veteran families with limited resources each month.
